Tremendous room for improvement. My mother went there after breaking her hip. Thankfully she is of sound mind. There were several occurrences where the nurses tried to administer the wrong medication. She could have been injured or worse. It was incredibly difficult to catch up with her case manager. My mother cannot eat pork. It was flagged as a dietary restriction—yet everyday she was served pork. The silver lining was the rehabilitation team. They are the unsung heroes of this place. Their dedication and communication were exceptional. My mom who is 76 y/o made significant progress in the time that she was there. Due to the administration staff and lack of attention to detail from the nursing staff—I’ll never send a love one there again. Scary.

I’m a former employee of 12 years . I really enjoyed working with the patients and the company . I sit and look at the complaints and bad reviews and say to myself that these people really doesn’t know what’s out there. I worked in so many different places via agency and will not dare step foot back in nor will I tell someone I even worked there. No place is perfect. They all have some pros and cons. But I believe in my heart that this place is alright. They do believe in ironing out the glitches... Let’s keep it 100%, a lot of companies lost staff due to COVID. Nurses and CNAS are chasing the money, or however wants to make their own schedule .... Then you have people that can’t even do COVID care. It’s going on every where. Be thankful to even have some staff anywhere.. OAN , let me know about that perfect place , I’m gonna go work there too , to see what the commotion is all about 👌

Worked through an agency as a nurse at Lexington of CR. I give much respect and high praise to the CNA staff who diligently and lovingly care for the residents. Outstanding with their ADL care and no excuses or complaints to providing showers as scheduled regardless to the workload that may be at hand. The CNAs hold high regard to assuring cleanliness and respect to the residents. A great team of CNAs that take pride in their assignments hats off to those I have encountered and worked closely with...

My mother is unfortunately doing rehab here. The Place is total trash. They’re completely run by agency staffing. Mother said she overheard nurses and staff talking about the covid cases, and i only see staffing wearing surgical masks. This place is a joke, dining services feed them jail food and no longer runs the way it did in 2015-16 when my mother was again doing PT. I cannot believe they’re allowing this place to run the way it is!

My mother was here recently. I moved her to be closer to home. She received good care here. The physical therapy team worked well with her, her medical needs were taken care of. My Mom spoke highly of a worker named Ms Debbie. My only suggestion is that the patients are taken outside for air and sunlight. I understand the Covid restrictions. But there should be a way for them to go outside a bit. Other than that. I like Lexington and would choose them again.

The object of a Rehab is to get well - as quickly as possible. This Center is making my recovery pleasant and quick! I fell Thursday, was rehab admitted on Friday and will be home in 12 days. All the people are professional, helpful and caring. Food is surprisingly good - soups are GREAT! PT/OT will push you to be all you can be. I made notes to help me be familiar with all the people I would see = 10 to 12 each day. One was nicer than the next. Each team member would ask how they could help - very refreshing in this day.

My Dad is currently there. Its more than difficult to see him because of Covid. Working in health care, I COMPLETELY understand this situation. However, they have shut his phone off, for the last 3 weeks. Even when he answers, they cut the connection...I hear his voice and someone says something and the phone is dead.

NOTHING BUT PRAISE!!!! My family is so grateful to the staff at Lexington Chicago Ridge. My Dad was there for 3 1/2 weeks which recently included the holidays. The staff was amazing! From the intake to admin- nurses- aides-doctors-NP-housekeeping-social service-discharge planner-dietary-PT and OT all very attentive. They were patient and professional not only to our Dad but to the entire family. Dad needed daily dressing changes and the wound care specialists were the best any of us had ever seen. They were a very thorough and knowledgeable team, and we thank you.We never used this facility before but decided to try this one when a different location we were familiar with was full. This will be our first choice going forward.Thank you for all of the care and attention you provided to Dad, it was a comfort to know he was with such caring people.

My mother was there the last few weeks of her life. The staff made her feel special, important, in other words, like a person. I will always be grateful for the care they gave her.

My grandmother was admitted for rehab to lessen her risk for a fall but has declined sharply since. She has dementia and anyone with a family member with this disease knows that you want to keep them able to do what they can for as long as they can. She was able to let us know when she needed to use the bathroom, she could feed herself okay, etc. Now, shes home and she cannot feed herself, barely eats and doesnt tell us when she needs to use the restroom. This is likely due to the fact that she wasnt checked on. At the beginning of her stay, she was able to say she needed to go and eventually would be put on a bedpan. She lost 20lbs while there for less than 2 months and is very weak. Its to the point where we may have to consider a permanent facility because she requires help for everything now. I cant help but wonder what the outcome of this stay wouldve been had she been put on the dementia floor and had staff that cared.I wouldnt recommend you send loved ones that cannot do for themselves here. Especially those with cognitive disorders that require extra assistance. We should have just brought her home after the hospital stay or found somewhere else.

Excellent place for my uncle. They really care at Lexington on 103rd and Southwest Hwy. I had no worries the entire time he was there for rehab. Great job guys😊👍🏼

My grandmother rapidly declined in this place and had to be taken to the ER. When she got there she was severely dehydrated, had c-diff, a severe UTI, and major kidney damage from the dehydration. I visited daily and kept trying to get them to get the required tests to help her. I could not even get them to give her Ensure even though she had not been eating most of the time she was there. They also flat out lied to me and said they had tested for the c-diff to make sure it was gone and they hadn’t. Two days I came in there and my grandma was thirsty and there was no water near her!! Finally I’d had enough and had her brought straight to the ER. I would not recommend this place if your loved one cannot fend for themselves. With that being said, most of the CNAs were nice.

This place can’t even pay their employees on time, I’ve been waiting for my check for 3 days now and still nothing. No returned phone calls from corporate or managers in Orland Park what a joke. Stay away bc this is the 3rd time we haven’t gotten paid.

The only complaint I have at Lexington health care center at Chicago ridge is. I have my mom from a post op surgery for rehab it been 5 days stay already I have seen neglected on patients you call for help and there is no answer at all. The only nurse that I see been more helpful to patients is Angela but she only works during the day. But during the night is more neglected . my mom had an accident she call for nurse took forever and when she finally came she didnt listen to my request to change her. Until Angela came at 7 am my mom was change. Worst help at this place. My family is all day with my mom. But when we leave she gets neglected

My father spent a few months in and out Lexington the year of his death (he died at home in hospice), the staff was always very nice and the room accommodating (he was contagious and had a room to himself). My father would have rather been home, but the facility was adequate. His real only complaint was the food.My mother was in Lexington late in 2013 and again in 2014 for about two months each stay with a chronic infection. The staff was wonderful, but they kept giving her roommates that were unsanitary (urinating on the floor from their bed to the bathroom), often exposing my mother to hazardous conditions and bacteria. You would think they wouldnt put someone with bodily function issues and poor hygiene in a room with someone with a chronic infection, especially when many other rooms are at half capacity/available.Also, the staff interrupts her many times during the night. She is there for IV therapy and rehab, there is no need to wake her 4-5 times a night to check her blood pressure and such (one morning someone was actually moving furniture around at 2am!!!).One of her biggest complaints, more so in 2014 since new ownership, is the food is horrible! Many times like dog food in smell, taste and appearance. Several times a week we bring her food and when we cant, she often passes on the meals (they dont credit you when you are not eating). This can not be good for her strength and rehabilitation efforts - probably prolonging her stay!They REALLY need to do something about the food they provide. Just because most of the residents are elderly does not mean you can feed them substandard food. I believe prisons feed their residents better. Also, be smarter on who you make roommates!

My mother was in for rehab and was treated horribly. When she went she had no bladder problems four days later they said she was incontinence, all because they never answered her calls. When we were there she had no problems getting up and going to the bathroom but overnight she never got any help. We would drop in late at night and she would be soaked in urine with her call button on nurses eating and gossiping at the nurses station. One nurse told us they didnt have time to read her chart?????? The rehab was a joke. I would never recommend this nursing home. I took my mother out and she 100% better at home. Horrible place.

A+ TEAM that worked with my mom , answered any and all questions, assisted us with going home care by showing us how to do basic functions that they perform. Went beyond the call of duty, KUDOS and A++++ to the following in no particular order. RN-LEENA, CNAS- DEBBIE, TEE, ERIKA, & BRIANNA, PHYSICAL THERAPIST- ROSE. THE ABOVED MENTIONED NEED PROMOTIONS AND RAISES. IF IT WERE UP TO ME IT WOULD BE DONE YESTERDAY. THEY WERE OUTSTANDING, MADE MOM FEEL AT HOME AS WELL AS MY SISTER AND I.

This a bad place to work and a even worst place to send your loved ones. They always mess up your pay and they leave residents in the bed wet all the time. They never answer call lights in a timely manner, the place has a ant problem, and they never treat residents with respect. The place looks nice but on the inside when residents family and guests not around its a different story. I would never recommend this place for no one to stay at or work at. Its a bad place to be at. DO NOT LEAVE YOUR LOVED ONES HERE YOU WILL REGRET IT!!!

The workers here are horrible, they don’t even care about the patients, they don’t do their beds or clean their bathrooms, for the patients that are bedridden, they are left alone and it’s just terrible! I would give them 0 stars but that’s not an option!

Lexington deserves no stars. How are patients supposed to heal and get better when you have staff that mistreat and berate them. My 92 yo mother was admitted with a left humeral fracture who had been having syncopal events. From the very first night, she was made to wait 20 minutes to use the bedpan. I myself witnessed that it would take not only waiting for 20 minutes but also walking to the nursing station to ask for help to have someone attend to my mothers call light. My mother was treated roughly by a particular CNA who had the audacity to tell her to hurry up in using the bedpan otherwise she would need to void in her diaper. When I approached the charge nurses to see what CNA had been mean to my mother, I was told they had no means of knowing what CNA could have gone into help my mom. It was very evident, the staff covered for each other. My poor mom expressed fear of being punished at night by this staff. Staff talked of how busy they were, yet they were seen sitting around on their phones or outside smoking a few feet from the front entrance. The condition of my moms room was terrible with water leaking from the air conditioner causing a potential fall. Messages left to the Director of Nursing and Administrator were unanswered. I am appreciative to Selena and Mary who were 2 nurses that appeared to exemplify compassion and sensitivity towards my mom. Lina and Jenn were 2 nurses with more seniority who seemed to empathize with our horrible experience at this facility. We are truly grateful to Ofelia who is a housekeeper who made my mother feel safe given that she spoke Spanish. I was told by a couple of the staff members that it would have been easier to care for my mom if she spoke English so they could understand her needs. I feel ashamed to know there are facilities such as this one. I would hope that people are appropriately screened and interviewed to work in these facilities. These patients are at the mercy of the staff. You have staff that should not work providing any patient care.

In my opinion Taking up to a half hour to respond to my bed ridden fathers help button is poor care. Their remedy is to discharge him under the disguise of not being able to help him. This came the day after I told them that this keeps happening and its unacceptable. In my opinion they are under staffed and patient care is severely lacking. I walked out to nurses station several nurses were standing around talking while my dads buzzer rang as if not to care. In my opinioni would never put a loved one in this facility again !!!!!!

They put my 91 year old mother out in March, 2015when her 20 days under Medicare ended without given me a chance to discuss payments due for the next 80 days at 20% out pocket expense . This was done based upon false infomations given to them by relatives who was taken money from my mother do to ELDERLY ABUSE. They did everything behind my back and refused to listen to me because they had made up their mind 4 days before I was informed. SERVICES WERE VERY PURE.

I worked here all of 2weeks. This place is nice as far as apperence but nothing else. My co woke was very unprofessional. The manage didnt care as long as they had workers. They have a high employment turn over rate. Just no a good place to work at. They treat they residents horrible on the 3 floor.

My husband was treated with excellent care.his medication was given on time.the did rehabilitation everyday if there was a change in his situation they would call.they always communicated with me.I feel comfortable with him coming back for rehabilitation.

Im writing this review in advance based on my tour of the facilities and speaking to two former patients (that I know personally) and one resident that is there now....I looked at two other facilities for placement of my Mom who fell and fractured her femur and hip...and chose Lexington based on my tour and personal reviews...but reading all of these negative reviews I hope I made the right choice for her...as I told the young lady who gave us the tour...If you fooled us you fooled us well! Shes being transferred in today...so well see....

They neglected and ultimately is responsible for the demise of my loved one. This occurred during the Covid-19 lockdown. There was no showering, grooming, or repositioning. As a result, a wound developed that was so deep in the flesh that it exposed the bones. I barely recognized my loved one when sent to the ER. Each night we called to check on their welfare and each night we were lied to.As a result, the infection spread and shut down several major organs. Per the medical doctors. Beware! They are liars.

May14,2019Day one, my mother was admitted today at 2pm. She had to asked for the dirty sheets to be removed from the bed.It is now 6pm. I walked into the room and my mother is in the bed without sheets . If I could give no stars I would.
